Application Process:

1. Download application form (website) or get it from the admission office (₹1,500 fee).

2. Fill the form and attach required documents:

   • Mark sheets and certificates (10th, 10+2, MBBS/MD/MS)

   • Migration & Character certificates

   • NEET UG/MHT-CET/MH PGM PGD CET/NEET PG/NEET SS documents (as applicable)

   • Caste/Disability certificate (if applicable)

   • Domicile & Nationality certificate

   • Medical certificate & Internship completion certificate (PG)

   • Registration as Medical Practitioner (DM & M.Ch)

   • Income certificates (fee waiver)

   • Photographs (4 passport size, 1 stamp size)

3. Submit the completed application form with all documents to the admission office.
